Here you will find all the release forms necessary to dive with us. These include release forms for Try Scuba, Medical Evaluations, Kids Programs, and Trips/Activities.

Medical

Please complete the medical evalution form below an return to our office prior to attending Scuba Training. If you check YES to any of the questions on the form, a doctors release will be necessary prior your training. If a doctor's release is needed, please take the Doctor's Guideline form with you to your appointment.

Try Scuba

Gobe's Try Scuba program allows interested individuals to "Get Wet" with one of our instructors before signing up for Scuba certification classes. Please complete both Try Scuba release forms and return to our shop prior to your Try Scuba session. If you answer "YES" to any of the medical questions in the Try Scuba Medical Release form, a doctor's release will be necessary prior to your Try Scuba session.

Kids Programs

We have several kids programs that introduce children to Scuba. We require parents to complete a Statement of Understanding to ensure parents understand the activites in which their children are participating.

Trips/Activites

If you are attending a pool session, or traveling with us to the Blue Hole or on a dive trip, please complete the following waiver. If you are joining us for a trip to the Blue Hole, please complete the Santa Rosa Permit Application.

Snorkeling

If you are attending a snorkeling session, please complete the following Snorkel Medical Form. If you answer "YES" to any of the medical questions, a doctors release will be necessary prior to your snorkeling session.
